What We Know So Far

Long COVID: What Is Established and What Is Still Being Studied

Long COVID research is moving quickly. To help you read with confidence, we separate findings that are widely accepted from ideas that are promising but not yet proven.

Well established

  • Long COVID is a recognized, infection-associated chronic condition. The U.S. National Academies define it as a condition present for at least three months after SARS-CoV-2 infection that affects one or more body systems.
  • It can follow mild as well as severe infections, and it can affect both adults and children.
  • Commonly reported symptoms include fatigue, post-exertional malaise (symptoms that worsen after physical or mental effort), brain fog, shortness of breath, heart-rate changes, sleep problems, and changes in smell or taste.
  • Symptoms may be constant, come and go, or worsen over time, and there is no single test that diagnoses Long COVID.

Emerging and under investigation

  • Researchers are studying several possible drivers, including lingering viral material, immune dysregulation, autoimmunity, reactivation of dormant viruses such as Epstein-Barr virus, small blood-clotting changes, and shifts in the gut microbiome. These may differ from person to person.
  • Some clinicians, including our educators, see overlap between Long COVID and illness linked to mold and other biotoxins, such as mast cell activation and immune imbalance. This is a clinical observation and an area of active interest, not an established finding.
  • Some people report persistent symptoms after COVID-19 vaccination. Researchers have begun studying these reports, and much is still unknown.
